RN Home Care Case Manager

Location: Warwick Rhode Island

Description: Pinnacle Home Care is in need of RN Home Care Case Manager right now, this vacancy will be placed in Rhode Island. More details about this vacancy opportunity kindly see the descriptions.

Experienced RNs to provide skilled nursing visits to patient's in their place of residence. Minimum of 1 years experience in home health PREFERRED. Experience in Med/Surg or acute care is a plus.
RN must be a graduate of an accredited professional nursing program and have a current RI license as a Registered Nurse.
Duties include OASIS assessments of patients, treatments, education, monitoring of medications and disease specific protocols.
RNs work in a team environment and case manage patients assigned to their caseload.
Pinnacle utilizes Point of Care electronic medical re! cord software, and provides teaching of the software.
