BAYADA HOME Health care has a immediate opening for an experienced energetic home health care Registered Nurse, RN for a Per Diem position as a home care Nurse case Manager providing 10-12 home visits from 830am-5:00pm for our office in Providence Rhode Island. Prior home health care experience strongly preferred but not required. Candidates who do not have home health care experience must have med-surg experience from a hosptial. As a home health care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities:
Making 10-12 home visits weekly to clients in designated throughout the state of Rhode Island.
Performing assigned duties, including administration of medication, would care, treatments and procedures
Monitoring clients conditions; reporting changes to Clinical and Client Service Managers
Follow up with, executing and properly documenting doctors' orders
Perfomring client assessments as necessary.
Strong candidate will have OASIS experience

Qualifications:
A current Rhode Island RN license. A minimum of one year of recent , verifiable clinical (medical/surgical) experience
A graduate of an accredited and approved nursing program as indicated by school transcript or diploma
Deomonstrated abliity to read, write and effectively communicate in English
Ability to work independently and manage time ef! fectively
Excellent organizational, interpersonal and communication skills
Solid computer skills; prior expereince with electronic medical records (EMR) preferred
Ability to travel to casses assigned with the territory
