Functions as a para-professional member of the Hospice team supervised by a Patient Care Manager. Perform various ser! vices for a patient as necessary to meet the patientâs perso! nal needs, to promote the patientâs comfort, and a safe environment. Observe patients and report/document these observations and the services provided. Manage related expenditures in a fiscally responsible manner in accordance with the Companyâs budget. Travel as necessary.
Requirements
- Minimum age requirement of 18.
- Completion of recognized Nurse Assistant certification program
- Passing score on Odysseyâs skills test.
- Minimum of one-year direct patient care experience in an institutional or home care setting required.
- Hospice experience preferred.
- Skilled in the proper technique for lifting and transferring patients.
- Ability to read, write and follow instructions and document care given.
- Understands Hospice philosophy and needs of terminally ill.
- Compliance with accepted professional standards and practices.
- Demonstrates tact, patience and good personal hygiene.!
- Excellent observation and communication skills.
- Ability to work within an interdisciplinary setting.
- Physical exam in accordance with state law requirements.
- Valid driverâs license with maintenance of a good driving record and reliable transportation that is insured in accordance with state and/or Odyssey requirements and is in good working order.
- Satisfactory references from employers and/or professional peers.
- Satisfactory criminal background check.
- Self-directed with the ability to work with little supervision.
- Flexible and cooperative in fulfilling all obligations.
