Position Overview:
Provide integrated health care and behavioral health care services. Develop professional and effective relationships with all stakeholders. Function as a team member and role model, demons trating effective interactions. Project a positive and professional image to the community as a representative of The Journey.
Principal Duties and Responsibilities:
- Screens and evaluates patients. Provides crisis intervention, medication management, psychosocial treatment and rehabilitation to patients.
- Provides care management and community integration services.
- Provides support services designed to assist individuals in addressing behavioral and health care needs.
- Employs a recovery-focused model of care which respects and promotes independence and responsibility.
- Promotes healthy lifestyles; provides prevention and educational services that focus on wellness and self-care.
- Supports individuals in self-management of chronic health conditions.
- Coordinates/monitors emergency room visits and hospitalizations, including participation in transitional/discharge planning and follow-up.
- Improves healt h outcomes of population served.
- Provides comprehensive case management and care coordination, including outreach, engagement, and triage based on acuity, assessment of service needs, and identification of gaps in treatment.
- Arranges and ensures access to person-centered plan, which includes access to primary care and other needed health care services.
- Schedules appointments.
- Monitors critical disease indicators.
- Employs prevention activities based on patientsâ needs, such as smoking cessation, substance abuse, increasing physical activity, obesity education, and chronic diseases (heart disease, diabetes).
- Ensures compliance with Federal and State regulations governing confidentiality.
- Participates in staff and treatment planning meetings. Supervises case managers; facilitates team meetings.
- Provides necessary outreach and patient-engagement strategies.
- Performs other related duties, as assigned. < /ul>
- Interpersonal communication skills
- Able to efficiently use a computer and Methasoft program.
- Holds basic understanding of alcohol/drug abuses and addiction.
- Able to develop professional, helpful and effective relationships with patients.
- Able to function as a team-member, working in conjunction with all other employees in a helpful, positive manner.
- Able to project a positive and professional image to the community as a representative of The Journey.
- Registered Nurse (RN ) (Required)
Minimum Qualifications:
Bachelorâs Degree or equivalent in Nursing from an accredited institution or Associateâs with RN License.
Knowledge, Skills, and Abilities:
Hours of Work: 40 hours/week, Mon-Fri, 5 a.m. - 1 p.m.
Compensation: Minimum starting rate $28/hour; full benefit package after 90 days of full-time employment
