POSITION TITLE : Manager of Care Management REPORTS TO : Senior Manager of Care Management DEPARTMEN T: Medical Management HOURS : 37.5 FLSA : Exempt

Overview:
The role of Manager of Care Management directs, and monitors all Case Management Programs, Clinical Programs and Health & Wellness Activities focusing on specific, targeted populations across all lines of business.

Required:

• Registered Nurse - Bachelor degree in health related field - 3-5 years clinical experi! ence - Strong analytical skills - Strong collaboration skills ! - Strong oral and written communication skills - Proven leadership skills - Proven ability to develop innovative programs and services - Customer service orientation - Strong interpersonal skills - Well-developed organizational and planning skills - Ability to independently problem solve Preferred : - Certification as a Case Manager from an accredited Case Management program - Master’s in health related field - 3 years managed care experience - 3 years public health/community health experience - 3-5 years operational management experience with proven commitment to organizational process - Bilingual a plus - Strong computer skills, including Microsoft Office applications - Knowledge of state agencies, community and family support resources Duties and Responsibilities: Responsibilities include, but are not limited to the following:
• Operationalizes all Case Management, Health & Wellness and Disease Management functions - Collaborates in the development, execution an! d evaluation of work plan for Care Management - Collaborates in the ongoing monitoring, evaluation, and outcome measurement for all Care Management Programs - Provides regular reports and presentations on outcomes of Care Management - Identifies opportunities and cost/benefit for Care Management Programs and enhancements to existing programs using data analysis tools - Collaborates in the development of targeted Care Management programs for new lines of business - Serves as Manager of staff required to administer program - Sets performance standards and productivity measures for direct reports - Professional development of staff, including identification of educational opportunities, annual performance appraisals, and goal development - Determines/monitors staffing ratios - Recruitment and retention activities for area of responsibility, including development of comprehensive role specific orientation programs - Develops policies and procedures for Care Management programs ! and ensures annual review and revision - Collaborates with Manager of C! linical Program Implementation and Development, Director, CMO, and AMD on new and existing Care Management programs - Collaborates in administrative budget development and monitoring for area of responsibility - Identifies and recommends opportunities for improvement and implements process improvement activities - Representative of Care Management programs with state and community partners - Other duties as assigned by Director - Collaborates with Matrix partners to develop, manage and evaluate programs Neighborhood is an EOE M/F/D/V and an E-Verify Employer.
