1 Full Time (40 hours); Monday-Friday 8:00 a.m.-4:30 p.m. with rotating weekend and holiday coverage, This position will be located in our Wakefield office and will be assigned the southern part of the state.
Description
The Registered Nurse (RN) provides skilled professional nursing care to the patient/family/ caregiver as prescribed by the physician and in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing, accrediting bodies, and the policies of Home ! & Hospice Care of Rhode Island. Case Manager RN’s have respo! nsibility to ensure that every patient’s individualized plan of care is developed, revised, and maintained throughout length of stay. Position reports to the Patient Care Manager.

  • Performs Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
  • Develops and implements the individualized plan of care and recommending revisions to the plan as necessary and plans for discharge/transfer as appropriate.
  • Coordinate all patient/family services and prioritization of needs with the members of the interdisciplinary team to develop and revise patient’s individualized plan of care.
  • Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care making adjustments to Plan of Care as needed.
  • Initiates and applies appropriate preventive and therapeutic nursing ! procedures and techniques, maintains communication regarding patient care in collaboration with the primary care providers and associated health professionals.
  • Facilitates the patient care with regard to the activities of daily living by promoting self-sufficient and optimal comfort care when appropriate. Teaches the patient/family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patents/families/caregivers.
  • Assesses home environment and teaches patient/family/caregiver regarding home environment/safety issues. Coordinates patient services with other team members to provide continuity of care. Works in cooperation with the family/caregiver and interdisciplinary team members to meet the identified needs of the patient/family/caregiver; including psychosocial, spiritual, and emotional needs and refers to appropriate ! disciplines/services. In facilities the care-giving team includes facil! ity staff.
  • Utilizes nursing assessment in establishing the certified nursing assistant care.
Bachelor of Science Degree in Nursing preferred; two years’ related experience or training; or related equivalent combination of education and experience. Home care, long term care, admissions and/or hospice experience preferred. A Rhode Island license in Nursing is required along with a valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order. Critical thinking skills are inherent to the position. The ideal candidate will possess strength in their ability to evaluate situations according to established standards and policies. In addition, ability to define problems collects data, establish facts and draw valid conclusions.
