Our MDS Coordinators, RNs, are specialists in their field and bring key skills to our centers and residents every day. SunBridge offers dedicated positions that do not include floor rotation, plus career advancement opportunities for MDS and clinical reimbursement management positions at our regional and divisional levels.
As MDS Coordinator you will p articipate in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). As well as, p articipates in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes; as required by Federal and State regulations.
MDS Coordinator, Registered Nurse, RN
Job Requirements :
- Must be a Registered Nurse, RN in good standing and currently licensed by the State
- Previous experience in a hospital, long-term facility, or other health care related facility.
- Experience in long-term care.
- Demonstrated proficiency in the RAI / PPS processes.
- Demonstrated knowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participation.
- Knowledge and competency on assessments, understanding RUGS optimization and ADLs.
- Previous experience in completing MDS’s and Care Plans is required.
- Strong communication, organizational and time management skills.
- Computer skills- Windows applications; computer experience; AccuMed experience preferred.
- AANAC Certification a plus.
