The Providence Community Health Centers, largest statewide network of community health centers, seeks a full-time, 35 hour/week Nurse Case Manager for The Connect Care Choice Program.
The Case Manager position provides comprehensive screening, assessment, care coordination and disease education to moderate-high risk adults with complex chronic health conditions in a community primary care practice setting.
Essential Responsibilities include the following :
Complete physical, medical and psych-social assessments for new program participants.
Incorporate RN Care/Case Manager role into health-center based clinic team to promote patient centered care, frequent contact with Primary Care Providers and medical home team members .
Promote, arrange and participate in optimal planned primary care physician visit including scheduling and arranging transportation.
Establish care plans, goals, interventions and contact based on risk category, patient/family members identified medical and social needs.
Support patient compliance with clinical outcomes by providing individuals with self-management supports including disease specific education materials, medication charts and side effects, signs/symptoms to monitor and report, nutritional recommendations, community support services/resources and care plan/treatment goals.
Act as liaison between health centers and patients to communicate admission information to facilitate dischare planning and follow-up care.
Attend meetings, participate in agency educational programs and community health events, if applicable.
Maintain professional ethics and confidentiality at all times.
QUALIFICATIONS:
Requirements include Bachelors Degree in Nursing, active RN License, 3 - 5 years experience in community health setting, behavioral health, chronic disease management, and/or case management.
Previous experience in case management, ability to work independently and manage large volume of cases.
Strong Communication skills: verbal and written. Ability to write routine reports/correspondence and speak effectively with patients, their families and employees.
