Job Description
The Case Manager will:
Manage appropriate levels of care by gathering information regarding the psychosocial care management plan
Develop relationships with providers, identified members, family and social support systems when indicated
Develop and facilitate implementation of the most cost effective plan of care for identified members while ensuring the highest quality of care
Required Skills
2+ years of post-Master's Degree experience in the delivery of counseling services in a behavioral health care setting
Master's Degree in related field
Clinical license in one of the following: Social Work, Professional Counselor, Marital & Family Therapist, Mental Health Counselor, or Registered Nurse
Customer oriented
Excellent communication skills
Highly organized
