JOB SUMMARY

The Nurse Care Manager will have the opportunity to work on and promote team-based interdisciplinary healthcare in the renal offices. H! e/she will be part of a program charged with implementing a Patient Centered Specialty Practice (PCSP) and administering health care in a new and innovative way. The Nurse Care Manager is responsible for providing comprehensive screenings, assessments, care coordination services, disease education and self-management support to the identified vulnerable populations. The Nurse Care Manager will be integrated into the office-based healthcare team to promote patient-centered care, frequent contact with primary care providers and PCSP team members, and actively participate in multidisciplinary patient-centered team meetings.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Consistently practices the UMF Customer Service Standards.
  • Follow established University Medicine EMR workflow protocols to maximize site’s
productivity and office flow efficiency.
  • Coordinate the care of targeted vulnerable patients with NCMs at primary ca! re sites according to care coordination guidelines set by phys! ician leads.
  • Provide detailed education about patient’s specific condition, including the pathology, signs and symptoms, complications, and medications used in treatment.
  • Assure that screening tests, immunizations, and urgent referrals are up to date for identified vulnerable patients. Perform outreach as needed when additional action is needed.
  • Utilize a multi-disciplinary team approach to address opportunities to plan and coordinate care.
  • Help to arrange contact with ancillary personnel.
  • Establish care management plans, interventions, treatment goals for targeted population; utilize motivational interviewing techniques to assist patients with establishing self-management goals and contact schedules on all vulnerable patients not currently at goal.
Ø Promote compliance with established patient care plan.

Ø Coordinate care and communicate with multiple providers, both within and external to the practice, w! ith particular attention to transitions of care.

Ø Review test results and tracks outcomes.

Ø Review medications and work with providers as needs to assist with medication management

Ø Review patient compliance and risk issues. Work with patient/family/care team to reduce risk.

Ø Work one-on-one with patients.

Ø Arrange group visits.

  • Leverage Pre-Visit Planning dashboard to prioritize patient outreach and scheduling.
  • Monitor clinical quality measures and manage clinical exception reports. Work with providers to achieve the measure targets.
  • Identify and utilize cultural and community resources to include motivational interviewing.
  • Ensure open communication, regarding patient status, with physicians and office staff.
  • Provide training to medical staff as needed.
  • Act as liaison to hospital, long-term care, specialists, PCPs and home health representatives.
  • F! ollow-up with all patients seen for an ER visit and/or hospitalization ! for continuity of care and to market enhanced access and reduce utilization.
  • Attend required training and collaboration sessions [i.e., learning sessions, outcomes congress, care management collaboration meetings, and practice team meetings] as scheduled.
  • Interact and coordinate with external Case and Disease Management staff, when applicable in caring for the patients within the Patient Centered Specialty Practice.
  • Perform quality work within deadlines within the renal office consistent with evidence-based treatment guidelines and NCQA PCSP recognition standards with or without direct supervision.
  • Share best practices among with NCMs at PCP sites, mentor and lead by example to support a positive work environment within UMF, and encourage other staff to do the same.
  • Represent the practice in a positive manner to all patients and all applicable external clients.
  • Bring issues to the appropriate manager(s) in a timely manne! r for resolution.
  • Perform other related duties as assigned.
  • Attend and be prepared for all internal meetings.
  • Host at least quarterly group patient education sessions.
  • Meet all required contractual obligations and visit/intervention requirements

  • Able to maintain confidentiality with all aspects of information (including patient data) in accordance with the practice’s philosophy and policy, and state and federal regulations. Must handle the most sensitive and confidential matters with the utmost discretion.
  • Profici! ency with computer skills (i.e., Microsoft Word, Excel and Access, and ! Web-based applications).
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:
Conditions common to a clinical practice environment including potential exposure to communicable diseases, medicinal preparations and hazardous materials. Involves frequent contact with patients and the ability to drive from site to site.

Education Requirements

  • Degree in Nursing from an accredited school.
  • Licensed RN, State of Rhode Island
  • Three (3) to five (5) years experience in outpatient or inpatient settings.
  • Experience working with patients regarding their care coordination and disease management / education is preferred.
