Home Care Licensed Practical Nurses

This is a very exciting time at Visiting Nurse Home Care --- we have achieved phenomenal growth and expect this growth to continue. Consequently, we need to hire additional Licensed Practical Nurses (LPNs) to assist in the care of our increasing patient census. Our LPNs provide skilled professional nursing care to the patient and family/ caregiver as prescribed by the physician and registered nurses care plan. As a LPN in the field, this position will supplement the nursing care needs of our patients and work in concert with other interdisciplinary team members, assessing home environment and teaching patient and family/caregiver.

• Knowledgeably and competently delivers nursing care to the patient in their homes
• Documents and charts appropriately using an electronic health record within specific time frames
• Follows physician ! orders and treatment plans for the patients
• Participa! tes effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner

Qualifications

• Graduate of Accredited School of Nursing
• Licensed Practical Nurse in the State of Rhode Island and Massachusetts
• Minimum of 1 year of clinical experience in a hospital or home healthcare setting
• Must be computer proficient; AllScripts experience ideal!
• Skilled in wound care, medication management, and assessment skills essential
• Valid driver’s license and auto liability insurance
• Reliable transportation to travel to patient homes per patient schedule and agency needs
• CPR license
• Ability to make independent clinical decisions
