Description: Life Care at Home is in need of Registered Nurse right now, this vacancy will be placed in Rhode Island. More details about this vacancy opportunity kindly read the description below. The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to: case management, specific nursing tasks, patient assessment, patient/family education,! clinical and progress note preparation, in-service participation, and coordination with the multidisciplinary team.

Knowledgeably and competently delivers nursing care to patients

Follow physician orders and nursing treatment plans for patients

Evaluates patient needs through ongoing assessment and revises care plans based on changes in patient condition

Oversees and evaluates care given by LPN and HHA

Identifies medical supply needs of patient and ensures patient has adequate supplies to meet care needs

Contacts physician and appropriately requests other disciplines be assigned to case; coordinates the development of plans of care with attending homecare physician

Regularly re-evaluates patients to identify progress towards goal achievement. Reviews OASIS answers, coding and sequencing of diagnoses to ensure accuracy; obtains orders and updates Plans of Care as needed throughout the episode of care

Do! cuments and charts appropriately and within specific time fram! es

Participates as part of an interdisciplinary team to assure that patient services are delivered in a timely and effective manner

Supports company's mission and values

EDUCATION and EXPERIENCE

Licensed as a Registered Nurse by the state in which practicing

Minimum of 1 year experience as a RN

Graduate of an approved school of professional nursing

Home Health experience preferred

CPR certification

SPECIFIC REQUIREMENTS

Valid drivers license and auto liability insurance

Reliable transportation to travel to patient homes per patient schedule and agency needs

Knowledgeable of nursing practices and procedures based on state practice acts

Ability to make independent clinical decisions

Able to (or assist with) lift, turn, move and position patients
