Description: Memorial Hospital of Rhode Island is looking of Nurse Care Manager right now, this job will be placed in Rhode Island. The Nurse Care Manager develops, plans and implements coordination of services for patients in collaboration with the multidisciplinary health care team. He/She will work with healthcare team ! with the full implementation of the patient centered medical home model within a primary care physician office. He/She will be required to work with both nursing/physician staff to facilitate to help achieve positive patient outcomes. He/she will be responsible to 1) perform comprehensive screening assessments, 2)care coordination, 3)disease education, 4)Self management support to adult patients with chronic health conditions (i.e.

CAD, depression, diabetes) and monitors compliance, 5) actively participates in multidisciplinary patient centered team meetings, 6)develops quarterly reports, 7)attends required training and collaborative sessions. Hours : 40 hours bi-weekly (1st shift) Education : Graduate of NLN accredited School of Nursing ; BSN required. Licensure : Current RI RN license required. Experience : Primary care office practice experience required.

Community nursing experience required. Bilingual in Spanish required. Certifications : Case managem! ent preferred. CDOE Certification within 1 year of employment ! required.

Special Qualifications: Ability to identify and implement components of the nursing process. Ability to communicate, collaborate and delegate effectively. Ability to work independently to achieve goals. Organizational and priority setting skills.

Ability to lift/move patients. Sit for extended period of time. Ability to travel via car. Self-motivation, commitment to continuing education and professional accountability.

Knowledge of community services, support agencies, government regulations. Proficiency with computer skills.
